On behalf of myself and my child, I hereby release Magic City Junior Golf Tour and its shareholders, directors, officers, employees, agents and contractors, including, without limitation, Neil Thompson and Ian Thompson, from all liability in connection with my child's participation on the Magic City Junior Golf Tour. I acknowledge that the activities conducted on the Magic City Junior Golf Tour may present a risk of injury/illness to my child, including as may specifically relate to Covid-19, and on behalf of myself and my child, I voluntarily assume such risk of injury/illness.
I, the parent or legal guardian of the above named minor child, hereby give my consent to the possible usage of their image, via social media or in promotion of the Magic City Junior Golf Tour, taken while participating on the Magic City Junior Golf Tour.
